ABSTRACT

Na Iniciativa Hospital Amigo da Criança-Neofoi proposto o uso da chupeta na Unidade Neonatal (UN) de modo terapêutico, e sempre com supervisão de um profissional de saúde. Mas observa-se que o aparato conhecido como "Luva Chupeta" fabricado com o uso de uma luva de látex está sendo utilizado como alternativa para acalentar o recém-nascido (RN). Apesar de poucos estudos, é evidente que esse dispositivo deve ser contraindicado na UN, uma vez que pode provocar alergia ao látex, transmitir infecções, provocar acidentes graves como aspiração laringotraqueal do algodão devido ao rompimento da luva de látex, e dependendo do tamanho do material, obstruir as vias aéreas, e levar a óbito. Além do mais, o dispositivo pode interferir negativamente no crescimento e no desenvolvimento craniofacial e causar prejuízos associados à amamentação e na saúde materno infantil. A substituição da "Luva Chupeta" por outras estratégias, até mesmo pela chupeta convencional ou ortodôntica, para lidar com a dor e situações de estresse do RN deve ser decisiva para evitar os riscos de acidentes graves. O Protocolo de uso de bicos, Protocolo de manejo da dor do RN, adoção do Método Canguru na UN para promoção do desenvolvimento e comportamento do RN, e a capacitação/monitoramento das práticas adotas pela Equipe Materno Infantil, quanto ao cuidado ofertado são alternativas mais complexas, mas que devem ser analisadas por aqueles que desejam oferecer confiabilidade aos seus processos institucionais. In the Baby-Friendly Hospital-Neo Initiative, the use of pacifiers in the Neonatal Unit (UN) was proposed in a therapeutic way, and always under the supervision of a health professional. However, it should be noted that the device known as "Pacifier Glove" manufactured using a latex glove is being used as an alternative to cherish the newborn. Despite few studies, it is clear that this device should be contraindicated in the UN, since it can cause allergy to latex, transmit infections, cause serious accidents such as laryngotracheal aspiration of cotton due to the rupture of the latex glove, and depending on the size of the material, obstruct the airways, and lead to death. Furthermore, the device may interfere with craniofacial growth and development and cause harm associated with breastfeeding and maternal and child health. The substitution of the "Pacifier Glove" for other strategies, even for the conventional or orthodontic pacifier, to deal with the pain and stress situations of the baby should be avoided to avoid the risk of serious accidents.The teat use protocol, the baby's pain management protocol, the adoption of the Kangaroo Method in the neonatal unit to promote the baby's development and behavior, and the training/monitoring of the practices adopted by the Maternal and Child Team, regarding the care offered, are alternatives more complex, but which must be analyzed by those who wish to offer reliability to their institutional processes. En la Iniciativa Hospital Amigo del Niño-Neo, se propuso terapéuticamente el uso del chupete en la Unidad Neonatal (UN), y siempre bajo la supervisión de un profesional de la salud. Pero se observa que el dispositivo conocido como "chupete Gluva", fabricado con el uso de un guante de látex, está siendo utilizado como una alternativa para cuidar al recién nacido (NB). A pesar de los pocos estudios, es evidente que este dispositivo debe estar contraindicado en la NU, ya que puede causar alergia al látex, transmitir infecciones, ocasionar accidentes graves como aspiración laringotraqueal de algodón por rotura del guante de látex, y dependiendo de la El tamaño del material obstruye las vías respiratorias y provoca la muerte. Además, el dispositivo puede interferir negativamente con el crecimiento y desarrollo craneofacial y causar daños asociados con la lactancia materna y la salud maternoinfantil. La sustitución del "Dummy Glove" por otras estrategias, incluso el chupete convencional u ortodóncico, para hacer frente a las situaciones de dolor y estrés del RN debe ser determinante para evitar el riesgo de accidentes graves. El Protocolo de Uso del Pezón, el Protocolo de Manejo del Dolor del RN, la adopción del Método Canguro en la NU para promover el desarrollo y comportamiento del RN, y la capacitación/seguimiento de las prácticas adoptadas por el Equipo Materno Infantil, en cuanto a los cuidados ofrecidos, son más alternativas eficientes, complejas, pero que deben ser analizadas por quienes deseen brindar confiabilidad a sus procesos institucionales. ABSTRACT

Objetivo: Analisar na literatura quais são os hábitos associados à mordida aberta anterior em crianças. Métodos: Realizou-se uma pesquisa bibliográfica referente aos artigos publicados de 2015 a 2020 nos bancos de dados on-line PubMed, Embase, SciELO, LILACS e BBO utilizando descritores e sinônimos MeSH, DeCS e Emtree com as seguintes etapas: identificação, triagem, elegibilidade e inclusão. Foram incluídos estudos observacionais, transversais ou de coorte, e artigos que demonstrassem hábitos associados à mordida aberta anterior em crianças de 2 a 12 anos de idade. Relatos de caso, revisões da literatura e outros tipos de estudos que não estavam de acordo com os critérios foram excluídos. Os artigos foram analisados por dois pesquisadores independentes e os estudos selecionados foram avaliados quanto à qualidade metodológica. Resultados: De um total de 462 artigos encontrados apenas 9 foram selecionados para o estudo. Desses, 8 (88,9%) apresentaram alta ou moderada qualidade metodológica, sendo apenas 1 (11,1%), dentre os artigos, de baixa qualidade. Mediante à análise dos estudos inclusos, observou-se que a manutenção de hábitos orais como sucção digital (66,6% dos artigos) e de chupeta (77,7%), bem como uso de mamadeira (33,3%) e duração do tempo de aleitamento materno (22,2%) pode ocasionar alterações na oclusão, fala, respiração, crescimento craniofacial, afetando diretamente a qualidade de vida da criança. Conclusão:Uma vez que a infância é a fase adequada para a implementação de novos hábitos saudáveis e tratamentos, é imprescindível que o cirurgião-dentista conheça essa associação, e adote medidas terapêuticas e preventivas.


Aim: To perform an analysis of in the literature regarding which habits are associated with anterior open bite in children. Methods:This was a bibliographic study conducted using articles published from 2015 to 2020 in the PubMed, Embase, SciELO, LILACS, and BBO online databases, using MeSH, DeCS, and Emtree descriptors and synonyms with the following steps: identification, screening, eligibility, and inclusion. Observational, cross-sectional, or cohort studies, as well as and articles demonstrating habits associated with anterior open bite in children aged 2 to 12 years, were included. Case reports, literature reviews, and other types of studies that were not in accordance with the criteria were excluded. The articles were evaluated by two independent researchers, and the selected studies were evaluated for methodological quality. Results:Of a total of 462 articles found, only nine were selected for the study. Of these, eight (88.9%) presented high or moderate methodological quality, with only 1 (11.1%) of the articles presenting a low quality. Through the analysis of the included studies, it was observed that the maintenance of oral habits, such as finger sucking (66.6% of the articles) and pacifiers (77.7%), as well as the use of a bottle (33.3%) and the duration of breastfeeding time (22.2%), may cause changes in occlusion, speech, breathing, and craniofacial growth, directly affecting the child's quality of life. Conclusion: Since childhood is the appropriate phase for the implementation of new healthy habits and treatments, it is essential for the dentist to understand this association and adopt therapeutic and preventive measures.

ABSTRACT

Objective: The aim was to evaluate the prevalence of malocclusion traits altogether and separate in anterior open bite, accentuated overjet and posterior cross-bite, and the possible association between them and sucking habits and breastfeeding. Methods : A preschool-based cross-sectional study was conducted with 472 children aged 24-60 months and their parents (response rate 86.6%). Three trained dentists (Kappa > 0.70) examine children for anterior open bite, accentuated overjet and posterior cross-bite and parents answered a questionnaire indicating the presence of breastfeeding, bottle usage, pacifier usage, finger sucking and the length of usage of all these habits. Results : The prevalence of malocclusion traits was 38.8%, Accentuated overjet was the most prevalent (30.3%) followed by posterior cross-bite (8.9%) and anterior open bite (7.4%). The unadjusted Poisson regression showed that children who were not breastfed had 63.0% more prevalent malocclusion traits when compared with those who were breastfed (RP 1.63, 95%CI 1.06-2.50). Length of pacifier usage in months was the only habit remained in the adjusted model associated with anterior open bite (RP 1.10, 95%CI 1.05-1.14, p < 0.000) and accentuated overjet (RP 1.03, 95%CI 1.01- .05, p = 0.004). For posterior cross-bite none habit showed association in the adjusted model. All models were adjusted for age and sex. Conclusion : Accentuated overjet is the most prevalent malocclusion trait. Length of pacifier usage is associated with the presence of malocclusion traits, anterior open bite and accentuated overjet. None of the investigated habits is associated with posterior cross-bite.


Objetivo: O objetivo foi estimar a prevalência de maloclusões avaliadas em conjunto e separadamente em mordida aberta anterior, sobressaliência acentuada e mordida cruzada posterior, e a possível associação entre elas e os hábitos de sucção e amamentação. Métodos : Estudo transversal foi realizado com 472 pré- escolares de 24 a 60 meses e seus pais (taxa de resposta de 86,6%). Três dentistas treinados (Kappa > 0,70) examinaram as crianças para mordida aberta anterior, sobressaliência acentuada e mordida cruzada posterior e os pais responderam a um questionário indicando a presença de amamentação, uso de mamadeira, uso de chupeta, sucção de dedo e tempo de hábito. Resultados : A prevalência de maloclusões foi de 38,8%. A sobressaliência acentuada foi o mais prevalente (30,3%), seguido da mordida cruzada posterior (8,9%) e da mordida aberta anterior (7,4%). A regressão de Poisson não ajustada mostrou que as crianças que não foram amamentadas apresentaram 63,0% maior prevalência de maloclusão quando comparadas com aquelas que foram amamentadas (RP 1,63, IC95% 1,06-2,50). A duração do uso de chupeta em meses foi o único hábito que permaneceu no modelo ajustado associado à mordida aberta anterior (RP 1,10, IC 95% 1,05-1,14, p < 0,000) e sobressaliência acentuada (RP 1,03, IC 95% 1,01- 1,05, p = 0,004). Para mordida cruzada posterior nenhum hábito mostrou associação no modelo ajustado. Todos os modelos foram ajustados por idade e sexo. Conclusão : A sobressaliência acentuada é a maloclusão mais prevalente. O tempo do uso de chupeta está associado à presença de maloclusão, mordida aberta anterior e sobressaliência acentuada. Nenhum dos hábitos investigados está associado à mordida cruzada posterior.

Abstract Objective: To investigate the association between pacifier use and bottle-feeding and unfavorable behaviors during breastfeeding. Method: A cross-sectional study was conducted with 427 babies/mothers. Socio-demographic, perinatal data, and information about the use of artificial nipples (pacifier and/or bottle) were collected through a questionnaire. The breastfeeding aspects regarding position, affectivity, sucking behavior, baby responses, and breast anatomy were evaluated through observation during breastfeeding. The chi-squared test and the multiple linear regression analysis were used to investigate the association between the variables. Results: The aspects of breastfeeding that showed higher percentages of the category "poor" were sucking behavior (22.5%) and position (22.2%). The group of infants who used pacifiers and/or bottle showed higher percentages in the poor and fair categories when compared with the good category for all five breastfeeding aspects evaluated (p < 0.001). The linear regression analysis revealed that the increase in the number of unfavorable behaviors regarding position, affectivity, sucking behavior, and baby responses were independently associated with both pacifier and bottle use (β positive, p < 0.05), while breast anatomy was independently associated only with bottle use. Conclusion: The findings suggest that the use of pacifiers and/or bottle-feeding may be associated with unfavorable behaviors during breastfeeding, especially the use of bottle-feeding.


Resumo Objetivo: Investigar a associação entre uso de chupeta e mamadeira e comportamentos desfavoráveis à amamentação durante as mamadas. Métodos: Um estudo transversal foi conduzido com 427 bebês/mães. Foram coletados dados sociodemográficos, perinatais e sobre o uso de bicos artificiais (chupeta e/ou mamadeira) através de questionário. Os aspectos de amamentação referentes à posição, afetividade, adequação da sucção, respostas do bebê e anatomia das mamas foram avaliados através da observação durante a mamada. O teste qui-quadrado e a análise de regressão linear múltipla foram usados para investigar associação entre as variáveis. Resultados: Os aspectos de amamentação que apresentaram percentuais mais elevados da categoria ruim foram a adequação da sucção (22,5%) e posição (22,2%). O grupo de bebês que usavam chupeta e/ou mamadeira apresentou percentuais mais elevados nas categorias ruim e regular quando comparados com a categoria bom para todos os cinco aspectos da amamentação avaliados (p < 0,001). A análise de regressão linear revelou que o incremento do número de comportamentos desfavoráveis referentes à posição, afetividade, adequação da sucção e respostas do bebê estavam associados de forma independente tanto ao uso de chupeta quanto ao uso de mamadeira (β positivo, p < 0,05), enquanto que anatomia das mamas estava associado de forma independente apenas com o uso de mamadeira. Conclusão: Os achados sugerem que o uso de chupeta e/ou mamadeira pode estar associado a comportamentos desfavoráveis durante amamentação, em especial o uso de mamadeira.

ABSTRACT

Abstract The aim of this article was to evaluate the influence of biological and sociobehavioral factors on the development of malocclusions in children. This is a cross-sectional study nested in a cohort of 350 children who participated in a community intervention study. After six years, an active search identified 290 children with occlusal abnormalities. Socioeconomic status, linear growth, and nutritional status of the child at birth and at the time of measurement, as well as dietary and sucking habits, were investigated. Malocclusions were evaluated across three spatial planes - anteroposterior, vertical, and transverse. The prevalence of these anomalies was 64.5%, primarily in the anteroposterior plane. The logistic regression analysis revealed that pacifier use for 60 months or more, stunting as measured at age six, and reduced gain in head circumference from birth to six years of age, were significantly associated with the development of malocclusion in childhood. Head circumference and linear growth are associated with occlusal anomalies in infants, independent of pacifier sucking.

ABSTRACT

Abstract This study aimed to evaluate the influence of breastfeeding and pacifier use on the occlusal status of preschool children. A cross-sectional study was conducted with children (n = 489) aged 2-5 years in private and public schools in Pelotas, South Brazil. Mothers answered a questionnaire on demographic, socioeconomic and behavioral variables, including breastfeeding and non-nutritive sucking habits. Malocclusion was classified as normal/mild and moderate/severe, according to WHO criteria. The association between breastfeeding and pacifier use with malocclusion was tested using multivariate logistic regression. The prevalence of malocclusion was 37.83% (95%CI 33.52-42.14), and 57.87% of the children used a pacifier. A total of 46.01% of the sample was exclusively breastfed for 6 months, and 24.74% were never breastfed. Malocclusion was associated with pacifier use, and with both non-exclusively breastfed and never breastfed. After adjustments for demographic and socioeconomic variables, the children who used a pacifier and were never breastfed (OR = 19.08; 95%CI 8.54-42.63), or who used a pacifier and were not exclusively breastfed (OR = 13.74; 95%CI 6.11-30.92), showed worse occlusal conditions, compared with the children who were exclusively breastfed and never used a pacifier. Pacifier use modifies the relationship between breastfeeding and occlusal status.

RESUMO Introdução As doenças e desordens bucais, bem como hábitos orais, podem causar impacto na qualidade de vida das crianças. Objetivo Associar a cárie dentária, maloclusão e hábitos orais com a qualidade de vida de crianças pré-escolares. Métodos Estudo transversal com 93 crianças de 3 a 5 anos de idade, acometidas, ou não, por lesões de cárie não tratadas. Os responsáveis responderam ao questionário sobre hábitos orais e ao Questionário sobre a Qualidade de Vida Relacionada à Saúde Bucal de Crianças na Idade Pré-escolar (B-ECOHIS). Um odontopediatra avaliou a gravidade das lesões de cárie e a presença de alterações oclusais. O nível de significância utilizado foi de 5%. Resultados O B-ECOHIS evidenciou impacto na qualidade de vida, conforme o aumento da idade, nos domínios dos sintomas, aspectos psicológicos, autoimagem e interação social e de função familiar. A cárie dentária apresentou impacto sobre a qualidade de vida das crianças e de seus familiares, especialmente em relação aos domínios dos sintomas e limitações. Hábitos de respiração oronasal e chupeta também evidenciaram efeitos negativos na qualidade de vida das crianças e de seus familiares. Não foi observada associação entre maloclusão e qualidade de vida relacionada à saúde oral. Conclusão A cárie dentária, os hábitos de sução de chupeta e de respiração oronasal demonstraram impacto negativo na qualidade de vida relacionada à saúde oral das crianças.


ABSTRACT Introduction Oral diseases and disorders as well as oral habits can impact the quality of life of children. Purpose To associate the dental caries, malocclusion and oral habits with the quality of life of preschool children. Methods Cross-sectional study with 93 children from three to five years of age who have or have not been affected by untreated carious lesions. Parents answered the questionnaire on oral habits and quality of life instrument related to oral health denominated Early Childhood Oral Health Impact Scale (B-ECOHIS). A pediatric dentist assessed the severity of the carious lesions and the presence of occlusal disorders. The level of significance used was 5%. Results The B-ECOHIS demonstrated impact on quality of life with increasing age in the fields of symptoms, psychological aspects, self-image and social interaction and family function. Dental caries were shown to have an impact on the quality of life of children and their families, especially in relation the domains of symptoms and limitations. Oronasal breathing and pacifier habits negatively impacted the quality of life of children and their families. There was no association between malocclusion and quality of life related to oral health. Conclusion The dental caries, the pacifier suction habits and oronasal breathing demonstrated negative impact on quality of life related to oral health of children.

RESUMO Objetivo: Verificar se o tipo de bico, ortodôntico ou convencional, de chupeta e mamadeira, tem alguma influência sobre as alterações encontradas no sistema estomatognático causadas pela manutenção do hábito de sucção. Estratégia de pesquisa: Por meio de uma revisão de literatura sistemática com metanálise, realizada a partir das bases de dados Lilacs, Medline e Embase e com a ferramenta de busca Google Acadêmico™, com os seguintes descritores em português e inglês: “Má oclusão” + “Chupetas”; “Má oclusão” + “Alimentação Artificial”; “Má oclusão” + “Mamadeira”, além das palavras “Bico Ortodôntico” + “Bico Convencional”. Critérios de seleção: Foram incluídos estudos que apresentassem em seus métodos a comparação de grupos que utilizaram chupeta e/ou mamadeira com bico convencional com grupos que utilizaram o bico ortodôntico, sem delimitação temporal. Análise dos dados: A análise do artigo na íntegra foi realizada de modo sistemático, com as seguintes categorias tabelando os resultados pertinentes: objetivo, método - casuística e avaliação, resultados e conclusão. Resultados: Foram encontrados 1.041 trabalhos, período de 1969 a 2013, desses foram excluídos 848 trabalhos, devido aos critérios de exclusão, e outros 174 que eram repetições. Foram lidos na íntegra 19 trabalhos, dos quais quatro artigos atenderam aos critérios de inclusão propostos, e três trabalhos foram incluídos na metanálise. Tais resultados expressaram que não há diferenças significantes entre os bicos ortodôntico e convencional quanto às implicações no sistema estomatognático. Conclusão: Não há possibilidade de concluir a existência de diferenças quanto às consequências no sistema estomatognático ocasionadas por bicos convencionais e ortodônticos de chupetas/mamadeiras.


ABSTRACT Purpose: Check if the type of nozzle, orthodontic or conventional, of pacifier and bottle have any influence on the changes found in the stomatognathic system caused by the maintenance of the sucking habit. Research Strategies: Through a systematic literature review with meta-analysis, performed from the databases Lilacs, Medline and Embase and Scholar Google, with the following key words in Portuguese and English: “malocclusion” + “Pacifiers “; “Malocclusion” + “Bottle Feeding”; “Malocclusion” + “Bottle feeding” beyond words “Orthodontic Beak” + “Conventional Beak”. Selection Criteria: We included studies that presented in their methods to compare groups who used pacifiers and/or bottle with conventional nozzle with groups using orthodontic nipple without temporal delimitation. Data Analysis: The analysis of the article in its entirety was performed systematically, ordering the relevant results in the following categories: objective, method—case studies and evaluation, results, and conclusion. Results: Found 1,041 jobs, from the period 1969 to 2013, 848 jobs were excluded based on the exclusion criteria and another 174 that were repetitions. A total of 19 articles were read in full of which 4 articles met the proposed inclusion criteria, and three studies were included in the meta-analysis. These results show that there are no significant differences between the orthodontic and conventional nozzles on the implications of the stomatognathic system. Conclusion: There is no way to conclude that there are differences as to the consequences to the stomatognathic system caused by conventional nozzles and orthodontic pacifier/bottle.

Introdução: o hábito de sucção pode causar alterações no sistema estomatognático devido ao desequilíbrio das forças que atuam naturalmente na cavidade oral. Quando este hábito se estende durante a mastigação e dentição mista, pode causar alterações no desenvolvimento e crescimento da face, afetando a dentição. Objetivo: este estudo teve como objetivo avaliar as alterações morfológico-funcionais causadas pelo uso prolongado da chupeta em crianças de 5 a 12 anos de idade, que possuíam tal hábito. Métodos: fizeram parte desta pesquisa 38 crianças, de ambos os sexos que frequentavam a Clínica da Criança e do Adolescente III e IV da Universidade de Passo Fundo/RS. Foi aplicado um questionário para as mães ou responsável a fim de investigar tempo de uso e características da chupeta utilizada. Resultados: destas, 22 apresentaram algum tipo de alteração, sendo a mais prevalente a mordida aberta anterior representando 57% das encontradas. Pode-se observar também mordida cruzada posterior, sobressaliência, desenvolvimento maxilar excessivo e ausência de selamento labial. Observou-se que as crianças que usaram chupeta até os dois anos de idade não apresentaram qualquer tipo de alteração proveniente do hábito de sucção. Conclusão: a idade em que o hábito é removido merece atenção por parte dos cirurgiões dentistas, alertando o momento apropriado em que o hábito bucal deletério deve ser removido.


Introduction: the sucking habit can cause changes in the stomatognatic system due to the imbalance of the forces that act naturally in the oral cavity. When this habit becomes extended during chewing and mixed dentition, it can cause changes in the development and growth of the face, affecting the dentition. Objective: this study aimed to evaluate the morphological and functional changes caused by prolonged use of pacifiers in children 5-12 years of age, who had the habit. Methods: they were part of this study 38 children of both sexes who attended the Clinic of Children and Adolescents III and IV of the University of Passo Fundo/RS. A questionnaire for mothers or responsible to investigate time of use and characteristics of the used pacifiers was applied. Results: of these, 22 had some kind of change, the most prevalent anterior open bite representing 57% of the found. One can also observe posterior crossbite, overbite, excessive jaw development and absence of lip seal. It was observed that children who used a pacifier up to two years old did not show any change from the sucking habit. Conclusion: the age at which the habit is removed deserves attention from dentists, alerting the appropriate time the deleterious oral habits should be removed.

Abstract This study aimed at evaluating the Protocol for the Prevention of Malocclusions (PPM), established in the preventive educational program developed by the Public Infant Oral Health Program of the State University of Londrina (PIOHP-UEL). Guardians of three-year-olds or older, maintaining nutritive (bottle) and/or non-nutritive (pacifier and finger) sucking habits, attended meetings designed to alert and guide them to eliminating these habits from their children. PPM patient records (2006-2013) were assessed and the data were described and evaluated by the Chi-square test, with a 5% significance level. Results 506 of the 802 guardians/children referred to the PPM joined. As for the children, the most frequently assessed habits were: bottle (56.1%), bottle and pacifier (18.4%), finger (11.9%), bottle and finger (7.1%), pacifier (5.7%), pacifier and finger (0.6%), and bottle/pacifier/finger (0.2%). After parent participation in the meetings, 335 (66.2%) children abandoned their habits. There was a statistical difference between type of oral habit and time to abandonment (p = 0.0001). However, those with only one habit abandoned it more easily (72.6%) than those with two or more associated habits (48.1%) (p = 0.042). Presence or absence of breastfeeding and parents' level of education had no significant effect on habit abandonment. Conclusion PPM was an important tool for spreading knowledge to guardians, greatly contributing to the abandonment of deleterious oral habits. Bottle sucking warrants special attention - mentioned by 81.8% of parents - either alone or associated with other habits. Thus, educational actions to implement the children's approach to oral health are fundamental to making behavioral changes and promoting education of healthy habits, thereby keeping malocclusions from developing.

ABSTRACT

The objective of this study was to evaluate the association of pacifier-sucking habit with occlusal and oral myofunctional alterations in preschool children. Eighty-four 2- to 5-year-old children participated in the study. Data on duration and frequency of pacifier use were collected from parents or guardians. Occlusal and oral myofunctional characteristics were examined by a dentist and a speech therapist, respectively. Chi-square tests and Poisson regression were used to analyze the data. The occlusal characteristics that were significantly associated with a pacifier-sucking habit were anterior open bite, altered canine relation, posterior crossbite, increased overjet, and malocclusion. The oral myofunctional characteristics that were significantly associated with a pacifier-sucking habit were resting lip position, resting tongue position, shape of the hard palate, and swallowing pattern. The strongest associations were for anterior open bite (prevalence ratio [PR] = 11.33), malocclusion (PR = 2.33), altered shape of the hard palate (PR = 1.29), and altered swallowing pattern (PR = 1.27). Both duration and frequency of pacifier-sucking habit were associated with occlusal and oral myofunctional alterations. These results emphasize the need for pediatric dentists to advise parents and caregivers about the risks of prolonged pacifier use and refer children to professionals for multidisciplinary assistance to minimize these risks whenever necessary.

OBJETIVO: Avaliar a influência de hábitos orais e do aleitamento materno sobre as habilidades orais de crianças. MÉTODOS: Estudo transversal que avaliou as habilidades orais de 125 crianças nascidas a termo, aos nove meses de idade, pertencentes à macrorregião centro-oeste do estado do Rio Grande do Sul, no período de agosto de 2010 a março de 2011. As variáveis estudadas incluíram avaliação das habilidades orais e informações sobre o aleitamento materno e a introdução da alimentação complementar. Análise de regressão logística simples e múltipla foi utilizada na análise dos resultados. RESULTADOS: O aleitamento materno influenciou positivamente a aquisição das habilidades orais de sucção das crianças aos nove meses de idade (OR 3,1; IC95% 1,2;8,3) e o hábito de usar a chupeta influenciou negativamente tais habilidades (OR 0,1; IC95% 0,03;0,6). CONCLUSÕES: O aleitamento materno contribuiu para o amadurecimento orofacial, pois melhorou a habilidade oral de sucção. O uso da chupeta mostrou alterar o funcionamento do sistema estomatognático. Os pais devem ser esclarecidos e recomendados a evitar o uso de chupetas durante a infância.


OBJECTIVE: The objective of this study was to investigate the influence of oral habits and breastfeeding on the oral skills of children. METHODS: Cross-sectional study evaluated the oral skills of 125 nine-month-old-children born at term, belonging to Macro-Midwest region of Rio Grande do Sul between August 2010 and March 2011. Variables included evaluating oral skills and information on breastfeeding and weaning. The results were analyzed using simple and multiple logistic regression. RESULTS: Breastfeeding positively influenced the acquisition of oral skills sucking at nine months of age (OR 3.1, 95%CI 1.2;8.3) and using a pacifier had a negative effect (OR 0.1, 95%CI 0.03;0.6). CONCLUSIONS: It was found that breastfeeding contributed to mature orofacial as it improved the ability of oral suction. Pacifier use was shown to affect the functioning of the stomatognathic system. This should be made clear to parents and the use of pacifiers during infancy should be avoided.

RESULTADOS: La lactancia materna influenció positivamente en la adquisición de habilidades orales de succión de los niños a los nueve meses de edad (OR=3,1; IC95% 1,2;8,3), mientras que el hábito de usar el chupete influenció negativamente tales habilidades (OR=0,1; IC95% 0,03;0,6). CONCLUSIONES: La lactancia materna contribuyó para la maduración orofacial, ya que mejoró la habilidad oral de succión. El uso del chupete mostró alterar el funcionamiento del sistema estomatognático. Los padres deben ser informados y recomendados para que eviten el uso de chupetes durante la infancia.

OBJECTIVE: To evaluate the association of breastfeeding duration, pacifier use and nasal air flow with occlusal disorders among children. METHODS: This cross-sectional observational study included 138 children aged 4 and 5 years selected in all the daycare centers of the city of Campo Limpo Paulista, Brazil. Questionnaires were applied to mothers to identify total duration of exclusive breastfeeding and non-nutritive sucking. The independent variables were: Sucking (pacifier, bottle, finger), breastfeeding duration and nasal air flow. The dependent variables were: Open bite, crossbite, overjet, overbite, diastema and maxillary deficiency. Frequency distribution calculations were performed, a chi-square test, the Fisher exact test and, after that, stepwise logistic regression were used for statistical analysis, and the level of significance was set at 5%. RESULTS: The use of a pacifier was the most deleterious factor and increased chances of having open bite in 33.3 times, marked overjet in 2.77 times and posterior crossbite in 5.26 times. CONCLUSION: There was a significant association between non-nutritive sucking, particularly the use of a pacifier, and occlusal disorders. These findings are important to plan the treatment of preschoolers.


OBJETIVO: avaliar a associação do tempo de amamentação natural, tempo de uso de chupeta e aeração nasal em relação a problemas oclusais em crianças. MÉTODOS: foi realizado um estudo observacional transversal com universo de 138 crianças entre 4 e 5 anos, de creches de Campo Limpo Paulista/SP. Foram aplicados questionários às mães com o intuito de identificar o tempo de amamentação natural total e exclusiva e hábitos deletérios. Utilizaram-se as variáveis independentes hábitos deletérios (chupeta, mamadeira e dedo), tempo de amamentação natural e aeração nasal; e as variáveis dependentes mordida aberta, mordida cruzada, overjet, sobremordida, presença de diastema e atresia. Foram realizados cálculos das distribuições das frequências, teste de associação do qui-quadrado, exato de Fisher e, em seguida, regressão logística pelo método de stepwise, utilizando nível de significância de 5%. RESULTADOS: o uso de chupeta foi considerado o mais prejudicial dos hábitos deletérios, aumentando em 33,3 vezes a chance de mordida aberta; 2,77 vezes a chance de apresentar overjet acentuado e 5,26 vezes as chances de apresentar mordida cruzada posterior. CONCLUSÃO: houve uma forte associação entre hábitos deletérios (especialmente o tempo de uso de chupeta) e problemas oclusais, sendo esse fato importante no planejamento do tratamento de pacientes pré-escolares.

Although malocclusions represent a serious public health issue, there is insufficient information about this problem in adolescents in Brazil, especially in poorer areas. The purpose of this cross-sectional study was to estimate the prevalence of facial alterations, dental malocclusions, and deleterious oral habits (DOH) among adolescents in a developing area in northeastern Brazil and to test the hypothesis that the occurrence of DOH in infancy is associated with DOH during adolescence. The study included a probabilistic population-based sample of 2,060 Brazilian students aged 12-15 years. Facial characteristics (type of facial profile, facial symmetry, and passive lip sealing) and malocclusions (Angle and Dental Aesthetic Index, DAI) were evaluated. DOH in infancy and adolescence were evaluated by interviews with the parents and adolescents. Most adolescents presented with normal facial characteristics. The malocclusion prevalence (Angle) was 83%. The DAI ranged from 13 to 69 (mean ± SD: 25.9 ± 7.7). Orthodontic treatment was necessary in 45.1% of the sample. The most prevalent DOH in adolescents were nail biting, object biting, cheek/lip biting, and bruxism, which were associated with finger sucking during infancy (P < 0.05). We conclude that malocclusions and DOH are common among Brazilian adolescents and that finger sucking during infancy may be a good predictor of DOH occurrence during adolescence.

ABSTRACT

OBJECTIVE: Nutritional, immunological and psychological benefts of exclusive breastfeeding for the frst 6 months of life are unequivocally recognized. However, mothers should also be aware of the importance of breastfeeding for promoting adequate oral development. This study evaluated the association between breastfeeding and non-nutritive sucking patterns and the prevalence of anterior open bite in primary dentition. MATERIAL AND METHODS: Infant feeding and non-nutritive sucking were investigated in a 3-6 year-old sample of 1,377 children, from São Paulo city, Brazil. Children were grouped according to breastfeeding duration: G1 - non-breastfed, G2 - shorter than 6 months, G3 - interruption between 6 and 12 months, and G4 - longer than 12 months. Three calibrated dentists performed clinical examinations and classifed overbite into 3 categories: normal, anterior open bite and deep bite. Chi-square tests (p<0.05) with odds ratio (OR) calculation were used for intergroup comparisons. The impact of breastfeeding and non-nutritive sucking on the prevalence of anterior open bite was analyzed using binary logistic regression. RESULTS: The prevalence estimates of anterior open bite were: 31.9 percent (G1), 26.1 percent (G2), 22.1 percent (G3), and 6.2 percent (G4). G1 would have signifcantly more chances of having anterior open bite compared with G4; in the total sample (OR=7.1) and in the subgroup without history of non-nutritive sucking (OR=9.3). Prolonging breastfeeding for 12 months was associated with a 3.7 times lower chance of having anterior open bite. In each year of persistence with non-nutritive sucking habits, the chance of developing this malocclusion increased in 2.38 times. CONCLUSIONS: Breastfeeding and non-nutritive sucking durations demonstrated opposite effects on the prediction of anterior open bite. Non-breastfed children presented signifcantly greater chances of having anterior open bite compared with those who were breastfed for periods longer than 12 months, demonstrating the benefcial infuence of breastfeeding on dental occlusion.

ABSTRACT

OBJETIVO: Verificar a eficiência de uma estratégia com atividades lúdicas de conscientização para remoção espontânea do hábito de sucção de dedo ou de chupeta em crianças. MÉTODOS: Participaram 15 crianças, nove do gênero feminino e seis do masculino, com faixa etária entre 4 e 8 anos (média de 5,6 anos). Foram desenvolvidas estratégias para a remoção espontânea dos hábitos orais com atividades lúdicas, realizadas uma vez por semana, com uma hora de duração cada, totalizando dez sessões. Ao final de cada sessão, os pais receberam a orientação semanal, esclareceram as dúvidas e relataram os comportamentos dos filhos durante a semana. RESULTADOS: Doze crianças (80 por cento) eliminaram completamente o hábito de sucção. Não houve diferença quanto à remoção quando comparados os hábitos de sucção de dedo e de chupeta (p=0,34). CONCLUSÃO: A estratégia de conscientização por meio de atividades lúdicas foi eficaz na remoção de hábitos de sucção de dedo e de chupeta em crianças.


PURPOSE: To verify the efficacy of recreational strategies of awareness for spontaneous removal of finger sucking or pacifier habits in children. METHODS: Fifteen children (nine female and six male), with ages ranging from 4 to 8 years (mean age of 5.6 years) participated in the study. Recreational strategies were developed for the removal of oral habits. The program consisted of ten one-hour weekly sessions. At the end of each session, parents were oriented, solved questions, and reported their child's behaviors during the past week. RESULTS: Twelve children (80 percent) totally eliminated the suction habit after the program. No difference was found for the removal when data regarding finger and pacifier sucking were compared (p=0.34). CONCLUSION: The awareness strategies using recreational activities was effective to eliminate finger and pacifier habits in children.

ABSTRACT

The aim of the present study was to investigate the prevalence of non-nutritive sucking habits and anterior open bite as well as their main causes (associated factors) in Brazilian children aged 30-59 months. A cross-sectional study was carried out during the National Immunization Day for polio in the city of Recife in the northeastern region of Brazil. The sample was made up of 1,308 children. Data were collected from interviews with mothers or guardians as well as from clinical examinations carried out by previously trained dental students. The chi-square test was used for statistical analysis at 5 percent significance level. The prevalence of non-nutritive sucking habits was 40 percent, and the habits were associated with gender (p=0.001), age (p=0.003) and feeding type (p<0.001). Anterior open bite was detected in 30.4 percent of children, and it was significantly associated with feeding type (p<0.001) and non-nutritive sucking habits (p<0.001). The variables found to be associated factors in the present study for the occurrence of non-nutritive sucking habits and anterior open bite emphasize the need to establish strategies that include orientation regarding health promotion based on the "common determining factors" approach. Public health policies should be adopted to encourage a longer duration of breastfeeding, thereby contributing towards reducing the prevalence of non-nutritive sucking habits and anterior open bite.


O objetivo do presente estudo foi avaliar a prevalência de hábitos de sucção não nutritiva e mordida aberta anterior, assim como, determinar os seus fatores de risco (fatores associados) em crianças brasileiras de 30-59 meses. Um estudo transversal foi realizado durante o Dia Nacional de Vacinação contra a pólio na cidade do Recife, situada na região nordeste do Brasil. A amostra foi composta por 1308 crianças. Os dados foram coletados a partir de entrevistas com as mães ou responsáveis, bem como, de exames clínicos realizados por estudantes de odontologia, previamente treinados. O teste qui-quadrado foi utilizado para análise estatística com nível de significância de 5 por cento. A prevalência de hábitos de sucção não nutritiva foi de 40 por cento, e os hábitos estavam associados com sexo (p=0,001), idade (p=0,003) e tipo de alimentação (p<0,001). A mordida aberta anterior foi detectada em 30,4 por cento das crianças. A mordida aberta anterior foi significativamente associada com o tipo de alimentação (p<0,001) e hábitos de sucção não nutritiva (p<0,001). No presente estudo, as variáveis consideradas fatores associados para a ocorrência de hábitos de sucção não nutritiva e mordida aberta anterior enfatizam a necessidade de estabelecer estratégias que incluam a orientação sobre a promoção da saúde baseada na abordagem dos "fatores de risco comum". Deste modo, as políticas públicas de saúde devem ser adotadas para incentivar uma maior duração do aleitamento materno, contribuindo, assim, para reduzir a prevalência de hábitos de sucção não nutritiva e mordida aberta anterior.

ABSTRACT

Objetivo: analisar uma possível associação entre a existência de hábitos bucais deletérios nos pais e existência de hábitos bucais deletérios nos filhos, observando o hábito de maior freqüência nestes grupos. Pretende-se, ainda, verificar a correlação entre a ocorrência de DTM e a presença de hábitos bucais deletérios nestas crianças. Métodos: aplicou-se um questionário a 50 crianças e seus respectivos pais na Clínica de Odontopediatria da FO/UFJF, abordando a presença e freqüência de hábitos bucais deletérios de sucção (chupeta e digital) e mastigatório (onicofagia, morder objetos, morder a mucosa oral, bruxismo e apertamento dentário), bem como a presença de quadros de desordens temporomandibulares nas crianças. Os dados coletados foram submetidos a analise estatística, sendo o qui-quadrado o teste escolhido.Resultados: Os resultados não demonstraram associação entre a existência dos hábitos bucais deletérios nos pais e a existência de hábitos bucais deletérios nos filhos, considerando o nível de 5% de significância (p>0,05). O hábito de morder objetos 74% (n=23) foi o hábito mais freqüente entre as crianças, enquanto que o hábito de apertamento dentário 46% (n=37) foi o de maior ocorrência entre os pais. Os resultados não demonstraram associação entre a presença dehábitos bucais deletérios e desordens temporomandibulares, independente do tipo de hábito.Conclusão: Os hábitos bucais deletérios são freqüentes na população infantil. Orientações e esclarecimentos são fundamentais nestes casos e capacitam os pais a se tornarem agentes estimuladores para o melhor desenvolvimento dos seus filhos


Purpose: To examine a possible association between the presence of oral habits in parents and the presence of oral habits in children, observing the habit of a higher frequency in these groups. The aim is also to investigate the correlation between the occurrence of TMD and the presence of oral habits in these children.Methods: A questionnaire was given out to 50 children and their parents at the Clinic of Pediatric Dentistry of FO/UFJF addressing the presence and frequency of oral habits of sucking (pacifier and digital) and chewing (biting nails, biting objects, biting the oral mucosa, bruxism and clenching), and the presence of tables of temporomandibular disorders in children. The data collected were subjected to statistical analysis, and chi-square test chosen.Results: The results showed no association between the presence of oral habits in parents and the presence of oral habits in children, considering the 5% level of significance (p>0.05). The habit of biting objects 74% (n=23) was the most common habit among children. While the habit of clenching 46% (n=37) was the most frequent among the parents. The results showed no association between the presence of oral habits and temporomandibular disorders, regardless of the type of habit.Conclusion: The oral habits are common in children. Guidance and clarification are essential in these cases and empower mothers to become agents for stimulating the better development of their children
